  • Even if you already manage diabetes patients every day, a fellowship gives you the chance to go deeper. You’ll see the direct impact of advanced, evidence-based care on patients while strengthening your medical knowledge and confidence in complex cases. The program gives you hands-on experience with the latest technologies and therapies and helps you become more comfortable leading interdisciplinary teams. Many fellows say the experience helps them connect the dots between science and everyday care, and that they leave with a stronger sense of purpose in treating diabetes. Graduates often become the “go-to” diabetes expert in their practice and someone that colleagues, patients and even payers trust for guidance. It’s an opportunity to grow as a clinician and help shape the future of diabetes care. 

  • While compensation may not change immediately, the fellowship is an investment in your long-term growth. You’ll gain specialized expertise, leadership skills and credibility that can lead to new career opportunities, including program development, teaching and health system leadership roles. As diabetology becomes more widely recognized across health systems and payer networks, those with formal fellowship training will be ahead of the curve. For most fellows, it’s not just about financial return, but about aligning their work with a deeper sense of purpose and impact.

  • Over the course of one year, you’ll gain comprehensive, hands-on experience across the full spectrum of diabetes care, including Type 1, Type 2, atypical, gestational and pediatric diabetes, as well as inpatient, outpatient and comorbid conditions such as cardiometabolic, renal, hepatic and mental health concerns. Every ACD-recognized program follows a standardized national curriculum while offering its own unique structure and culture. We encourage applicants to explore each program’s focus to find the best fit for their goals and interests. 

  • Absolutely. Technology is at the heart of modern diabetes care, and it’s woven throughout the fellowship experience. Fellows get direct, hands-on training with continuous glucose monitors, insulin pumps, connected devices and emerging digital platforms. You’ll learn how to interpret complex data and use it to guide real-time treatment decisions, helping patients achieve better control and quality of life. This foundation also positions you to stay on the leading edge as new tools and therapies continue to evolve. 

  • Not at this time. Because the fellowship involves direct patient care and hands-on training with advanced technologies, it must be completed in person at a participating program. The immersive, team-based environment is part of what makes the experience so valuable. You’ll work closely with patients, educators, pharmacists and nurses who form the backbone of effective diabetes management. 

  • We’re in the process of pursuing ACGME accreditation. The accreditation process takes several years and requires data from multiple graduating classes to demonstrate strong outcomes. Our current structure is built to those same high standards, and what we learn from our fellows continues to shape that journey. The ultimate goal is to ensure diabetology is fully recognized within the medical community as a distinct and essential subspecialty. 

  • The outlook for diabetology is incredibly strong. Diabetes is one of the fastest-growing and most complex health challenges in the U.S., and care is becoming more technology-driven and interdisciplinary every year. Physicians with advanced training in diabetology will be in high demand to lead care teams, manage complex populations and guide health systems in improving outcomes. It’s a field with long-term relevance and enormous potential, and those who step into it now will help define what the next generation of chronic disease management looks like. 

  • Physicians who have completed an internal medicine, family medicine or primary care related residency are eligible to apply.
